Four Judicial Council forms containing figures based on the federal poverty guidelines need to be revised to reflect the changes in those guidelines recently published by the federal government.

Recommendation
Staff of the Judicial Council recommended that the Judicial Council, effective March 1, 2016, revise the following documents to reflect 2016 increases in the federal poverty guidelines:
• Request to Waive Court Fees (form FW-001);
• Request to Waive Court Fees (Ward or Conservatee) (form FW-001-GC);
• Information Sheet on Waiver of Appellate Court Fees (Supreme Court, Court of Appeal,
• Appellate Division) (form APP-015/FW-015-INFO); and
• Financial Declaration-Juvenile Dependency (form JV-132).
